This is what is needed by law to qualify a person for this job using these pieces of equipment. If an employee can not physically do what is requested and you can not make reasonable accommodations and you described these physical/emotional requirements then that person is not capable of doing the job required of them.
If an employee fraudulently says that he or she can do the job and that he or she has no previous injuries that would relate to the Physical Standard described to them on the Codes of Safe Practices© then they are committing fraud. And believe it or not as an employer you have some rights too!
